Fill out the QuadLife repair form, including the unit serial number and a brief explanation of the problem.
The $230.00 charge for the repair is a flat rate and will include physical damage as long as the circuit board is not damaged. If the unit is in warranty and has not been physically damaged, the $230.00 charge will be refunded.
If the circuit board is physically damaged or an out of warranty unit cannot be repaired the $230.00 charge can be refunded or credited toward a new unit along with a $230.00 credit toward its purchase (damaged unit will not be returned).
